Disability of the beneficiary:

Notification of the award of a 2nd or 3rd category disability pension
or a certificate from the Regional Social Security Fund specifying that you are classified in 2nd or 3rd disability category.
or a copy of the disability card issued by the CDAPH (subject to recognition of a disability rate of at least 80%),
or a photocopy of the mobility inclusion card with the mention “disability”.
or the last notification of allocation of an old-age pension (retirement) for medical incapacity for work issued by Social Security to the disabled person aged over 60.
or Certificate of disability for the RSI scheme for self-employed workers

Disability of spouse, PACS partner or children:

Documents identical to the case of disability of the beneficiary.
And Copy of the up-to-date family record book or copy of the extract of the marriage certificate or copy of the extract of the birth certificate with mention of the PACS.

AND

The completed and signed release form which will be sent to you
A double-sided photocopy of your valid proof of identity, dated and signed by you
A copy of your membership certificate or, failing that, a sworn attestation of the loss of this certificate
A bank account number.
