Declaration on honour of the applicant's family relationship with the person receiving assistance or of the assistance provided to an elderly or disabled person with whom he or she resides or maintains close and stable ties.

And depending on the case:
- When the person receiving assistance is a disabled child dependent on the applicant, within the meaning of Article L. 512-1 of the Social Security Code, i.e. who lives at home and is under 15 years old on 30 April of the current year, or a disabled child who lives at home regardless of their age, or a disabled adult: a copy of the decision taken in application of social security or social assistance legislation subject to proof of a permanent disability rate of at least 80% ;
- When the person receiving assistance suffers from a loss of autonomy : a copy of the decision to award the personalized autonomy allowance (APA) mentioned in article L. 232-2 of the social action and families code.
- When the person benefits, a copy of the decision to award the following benefits:
  - Increase of a third person (MTP) for constant assistance;
  - Additional benefit for recourse to third parties;
  - Special increase for assistance from a third party covered by the National Retirement Fund for Local Government Employees (CNRACL);
  - Increase awarded to disabled civil servants and magistrates required to have recourse to the assistance of a third party for ordinary acts of life;
  - Third-party increase for military personnel and war victims.
Or the certificate from the beneficiary's employer or the employer of their spouse or civil partner), indicating that they are receiving leave to care for their loved ones .
